The Information Navigation service provides advice and information on all areas of health and social care. Whether you are receiving services from the local authority or funding your own care we can help support you through the health and social care system.

How the service works

The aim of the service is to help people to live as independently as possible within the community. You can tell us about your situation and together we can identify the services available to help you.

We can also help you to access a huge range of other activities and services in the borough including; benefits, money, debt, and housing advice, leisure opportunities, children and families services, grants, health and social care services.

The service is for all adults including people with; long term conditions, learning difficulties, mental health, neurological conditions, disabled people and older people.

We can also help if you need information for a friend, neighbour, or someone you are caring for. We take referrals from GP’s, social workers and other professionals. Or, you can contact us directly! We would love to hear from you.

If you are interested in finding out more we can give you information, advice and support by phone, email, home visit or by appointment at our Information Hub.
